Community Health Programs Coordinator at Pearl Hospital

Overview​

Pearl Hospital is seeking a motivated and experienced Community Health Programs Coordinator to lead the implementation of its community-based health initiatives. The role 

involves coordinating field activities, managing community health staff, and ensuring effective  delivery of care to clients outside the hospital setting. 

Key Responsibilities 

• Plan and implement community outreach activities including community NCD and home-based care programs in collaboration with key partners 
• Coordinate and supervise field teams providing health services at community and household level 
• Support patient follow-up, care continuity, and reporting 
• Strengthen linkages between hospital services and community health structures 

Qualifications: 

• Degree or Diploma in Nursing, Community Health or related field 
• Minimum 2 years’ experience in community health programming 
• Strong organizational and leadership skills 
• Experience managing field-based teams
