    Baseline Evaluation Terms of Reference USAID Msingi Imara Project

    Project Objectives and purpose
    The overarching purpose of the Msingi Imara project is to strengthen the Human Resources for Health, Quality Management and Community Health Systems. It aims to build on existing interventions, identify gaps, and enhance maturity of the supported systems towards sustainability and in the journey to self-reliance. To achieve this purpose, it focuses on three sub-purposes:

    • Sub-purpose 1: Strengthen Human Resources for Health management systems.
    • Sub-purpose 2: Improve quality management systems, and
    • Sub-purpose 3: Strengthen Community Health Systems at National and County levels.

    Project Theory of Change

    • USAID Msingi Imara’s theory of change is premised on the strong conviction that if our partnership collaboratively uses innovative and highly proven strategies to address systems and contextual gaps in Human Resources for Health system, quality of health service and community health systems and empower focus counties to increasingly take up these innovations, then it will ultimately contribute to improved sustainable health outcomes in the focus counties.

    Needs based training, rational deployment, appropriate utilization and supervision, and cordial employer-employee relations are critical in addressing the human resources for health constraints. The project will focus on building the capacity of national and sub-national (county) human resources for health (HRH) management systems to address issues pertaining to health worker productivity, retention, and quality of services delivered. This project will be in line with the following guiding principles: a system thinking and systems practice approach; respect of the country's devolved system of health governance; local capacity development, building on and strengthening the capacity of local entities in the public and private sector; promoting system change and sustenance of results. The principles have been mainstreamed in the technical approaches highlighted below:

    Strengthen Human Resources for Health management systems.

    • Strengthen Health Workforce Performance Management
    • Improve Workplace Environment
    • Strengthen HRH management deployment and retention.

    Improve Quality management systems.

    • Strengthen Quality Planning
    • Strengthened Quality Assurance
    • Strengthening Quality Improvement

    Strengthen Community Health Systems at National and Community Levels

    • Strengthen health systems for improved quality of community health services.
    • Strengthen community health systems to enhance equitable distribution of services.
    • Strengthened community health systems for enhanced optimization of resources.

    The baseline objectives
    The baseline evaluation will contribute towards:

    • Establishing a baseline as points of comparison to support measurement of project impact as part of future project evaluation activities.
    • Guiding realistic and feasible target setting for performance indicators where needed
    • Informing the planning and implementation process including MEAL systems
    • Understanding of the operational environment and existing priorities in the target counties

     

    The general responsibilities are:

    • Review of program documents and preparation of inception report and data collection tools
    • Review of secondary literature to inform the development of data collection tools.
    • Develop the baseline data collection schedule and select respondents to participate in interviews.
    • Select and Lead training of data enumerators/research assistants.
    • Conduct baseline data collection.
    • Perform data quality checks to ensure accuracy, validity and integrity of data collected.
    • Conduct data cleaning, processing and analysis.
    • Prepare and submit a draft baseline report and other products.
    • Validate findings with the project team and project partners i.e., preliminary Presentation of Report
    • Submit final baseline report and annexes.
    • Participate in validation/dissemination event.

    Roles and Responsibilities of consortium partners

    • Seeking of requisite approvals, including IRB approval as appropriate
    • Onboarding meeting to explain scope and align expectations, agree on logistics etc. Another touch point after data collection and at presentation of draft report (to include technical officers in the project)
    • Provide the required project information to the consultant, these include project proposals and log frames and any type of research already performed on the target groups.
    • Provide technical review of inception reports, data collection tools and quantitative data.
    • Approval of inception report, tools, quantitative data formats and data collection schedule
    • Mobilization of relevant stakeholders to participate in interviews or FGDs.
    • Participation in the data collection as informants where applicable
    • Technical review and providing comments on draft report.
    • Mobilization of stakeholders to participate in validation/dissemination event.
    • Approval of final report and annexes

    Qualifications of the proposed consultants
    Proposed consultants should meet the following requirements at a minimum:

    • Lead consultant should have at least a master’s degree in epidemiology, public health, biostatistics or related field. Other team members should have qualifications in Human Resources for Health, health systems, Primary Health Care (Community Health), data analysis and quality improvement.
    • At least 7 years of experience in research, and monitoring & evaluation of which a minimum of 5 years of related research experience, including both academic and project-oriented research.
    • Experience in developing sampling plans and tools for and undertaking Focus Group Discussions, Key Informant Interviews, and Surveys
    • Experience of working in the humanitarian sector preferably in health system strengthening
    • Experience conducting evaluations/baselines for USAID and other relevant donor projects.
    • Previous experience evaluating government capacity building and policy reform initiatives.
    • Strong qualitative and quantitative research and analytical skills.
    • Experience of working with multicultural teams
    • Experience of working in the region or counties of the project.
    • Language skills – Proficiency in English and Kiswahili.
    • High level of professionalism and an ability to work independently and in high-pressure situations under tight deadlines.
    • The lead consultant must have strong analytical skills and ability to clearly synthesize and present findings, draw practical conclusions, make recommendations and to prepare well-written reports in a timely manner.
    • Availability for the period indicated and ready to carry out the assignment and deliver results within the specified period/time.

    Peer Coach - Vihiga

    Job Purpose

    • To primarily coach and mentor the Community Health Extension Workers (CHEWs) & Community Health Assistants (CHAs) in supporting a network of Community Health Promoters (CHPs) within a sub county to deliver high impact through best practice, digitally enabled performance management and supervision, therefore, contributing towards the delivery of impactful community health by the sub county and county health teams.

    Key Responsibilities / Duties / Tasks

    • Coach and mentor CHAs & CHEWs attached on performance management, use of digital tools, utilization of dashboards and use of data for decision making.
    • Support in strengthening the linkage of Community Health data by building on strong facility engagement in community health unit activities and data transmission into the Kenya Health Information System.
    • Accompany CHAs & CHEWs for Field Visits and offer support supervision. 
    • Undertake necessary measures to ensure that CHAs & CHEWs and attached CHPs report quality data.
    • Offer first level support on Android issues faced by the CHAs, CHEWs & CHVs.
    • Participate in Community Events including Community Health Unit meetings, Community dialogue & Community Action days.
    • Support in-service training & continued refresher trainings to CHAs, CHEWs & CHPs, and any other trainings that may arise. 
    • Establish networks and support CHAs & CHEWs in strengthening their working relationship with facility staff, CHC and other partners to collaboratively improve community health service delivery.
    • Develop monthly activity plans to achieve objectives and document key learnings, best practices and share for continuous improvement on program delivery.
    • Contribute to planning and implementation of Sub-County level performance improvement plans.
    • Analyze, interpret, and present their individual/sub-county performance.
    • Support CHAs & CHEWs in quantification, forecasting, management and reporting of Community Health Unit commodities. 
    • Develop and maintain a good working relationship with local administrative government in the area of operation including Community Health Committee, Village Elders, Sub Chief/Chief etc.
    • Attend and support CUs review meetings and sub county planning meetings. 
    • Alert the Supervisor Coach to any arising risks, challenges, and impediments to implementation.
    • Perform any other duty deemed necessary or as delegated by the Supervisor coach. 

    Academic Qualifications & Work Experience

    • Degree or Diploma in community health, public health, business management or any related field
    • Having worked in the community health space at county or Sub-county level for at least one year.
    • Minimum of (1) year relevant experience for degree holders and (2) years relevant experience for diploma holders as a Community Health Supervisor in community health field.

    Competencies/Attributes:

    • Strong organizational and planning skills.
    • Good data analysis skills.
    • Learns from experience and applies what is learned to new situations.
    • Plans, prioritizes, breaks down tasks accordingly.
    • Supports in Program/projects in data analysis, collation of information, reporting etc.

    Supply Chain Coordinator - Isiolo

    The Role

    • Work closely with the County and Sub County pharmacists and other relevant government officers to ensure CHVs are in stock. Train county and sub county pharmacists on medicine forecasting and track sub-county pharmacist forecast submissions. Ensure all adverse effect reactions are consigned and reported by sub-county pharmacists as per expectations. Provide support in-field to assigned CHAs / CHEWs when necessary, as identified by supervisor coaches

    Key Responsibilities / Duties / Tasks

    • Government Relations
    • Ensure health facilities and CHVs are always in stock of essential medicine. 
    • Work closely with the County and Sub County pharmacists and other relevant government officers to ensure CHVs are in stock.
    • Train county and sub county pharmacists on medicine forecasting and track sub-county pharmacist forecast submissions.
    • Engage relevant county and sub-county personnel to ensure medicine is set aside for CHVs at health facility level. 
    • Ensure CHVs are restocked in a timely manner, and follow-up with relevant personnel when not. Escalate concerns to the Government Relations team.
    • CHP support
    • At assigned branches or sub-counties, monitor the dispensing of medicines by CHVs in the field to ensure compliance of all treatment and dispensing protocols and undertake pharmacovigilance.
    • Support the branch and sub-county team in the delivery of health training during CU trainings and support the training teams during base trainings within their county. 
    • Provide support in-field to assigned CHAs / CHEWs when necessary, as identified by supervisor coaches.
    • Staff and CHA training and coaching
    • Deliver continuous skills training on relevant health topics and coach staff and CHAs within their county to deliver high quality community health work.  Advise the Pharmaceutical and Procurement Manager on areas where additional support is needed. 
    • Gather and share best practice in health and pharmaceutical management within their branches or sub-counties and more widely across Living Goods.
    • Pharmacovigilance / adverse effect reactions / compliance to treatments
    • Ensure all adverse effect reactions are consigned and reported by sub-county pharmacists as per expectations.
    • Ensure county and sub-county pharmacists focus on compliance to treatments by CHVs, and keenly follow-up on any potential challenge identified by the QC teams.

    Academic Qualifications & Work Experience

    • Diploma in Pharmaceutical Technology required (NOTE: must be registered with Pharmacy and Poison Board) and have your license available for use.
    • Professional Qualifications / Membership to professional bodies
    • Pharmaceutical professional trainings
    • Minimum 2 years’ experience in health or community related work.

    Competencies/Attributes

    • Ability and willingness to work extensively in the field – Isiolo. 
    • Ability to interact with community elders, Community Health Volunteers, and clients.

    Supervisor Coach - Vihiga

    The Role

    • To build competencies of Peer Coaches to ensure coaching and mentorship of the Community Health Extension Workers (CHEWs) & Community Health Assistants (CHAs) in supporting a network of Community Health Promoters (CHPs) within a sub county to deliver high impact through best practice, digitally enabled performance management and supervision. This is aimed at contributing towards the delivery of impactful community health by the sub county and county health teams.
    • To lead and provide direct supervision and support to sub county field-based teams and oversee all operations within the sub county.

    Key Responsibilities

    • Provide technical support by conducting trainings and coaching sessions to community health extension workers (CHEWs), Community Health Assistants (CHAs) and support sub-county health management teams (SCHMT) on performance management and support supervision of CHPs.
    • Provide ongoing coaching of CHEWs/CHAs on how to review and analyse data on the performance dashboards and interpret to support CHPs to deliver against targets.
    • Provide technical assistance and capacity-building on service delivery, supply chain management, digital health, management of community health human workers, and compensation of CHPs, among others, and share learnings and get feedback.
    • Accompany and Support CHEWs/CHAs and other subcounty health teams on field visits.
    • Support the Sub County Community Health Strategy Focal Person (SCCHSFP) in all engagements and co-design at sub county levels, helping to establish ways of working to ensure the sub county is fully engaged.
    • Support the SCCHSFP to monitor performance based on milestones demonstrating that, Living Goods and the County Government can achieve the set performance metrics sustainably over time.
    • As needed, represent, and offer support in sub county technical meetings and engagements and actively seek out, identify, advise upon, and implement opportunities for collaborative work across the health sector.
    • Support CHEWs/CHAs prepare and present community units level reports, presentations, and key insights, as necessary. 
    • Develop and initiate new ideas to improve how we provide technical assistance.  
    • Lead in Sub-County level documentation of lessons learned and best practices to leverage on learnings to inform government planning on the capabilities needed for current and future requirements to maintain a functional eCHIS.
    • Participate in Sub-County level development of AWPs and advocate for incorporation of DESC elements for effective implementation of a functional eCHIS.
    • Strengthen networks and linkages with public, and other implementing partners involved in community health programming.
    • Alert the Program manager to any arising risks, challenges, and impediments to implementation. 
    • Perform any other duties assigned by the Program manager.

    Academic Qualifications & Work Experience

    • Degree in health sciences.
    • Post-graduate training in Management, Public or community Health is an added advantage.
    • 5 years or more with sound understanding of the Kenya Health System and prior technical assistance work experience with and/or involving close association with national and county governments. 
    • Experience supervising teams
    • Experience working with community health workers in rural villages

    Competencies/Attributes

    • Strong interpersonal, communication and report writing skills.
    • Should be flexible and willing to travel across Kenya and primarily be based in the field.
    • Strong analytical skills is a mandatory. 
    • Ability to prioritize tasks, take initiative, and work without constant supervision. 
    • Honest, reliable, diplomatic, and well organized.
    • Must be computer literate with proficiency in MS office suite
    • Collaborative team player.
